Syllabus
Student Learning Outcomes, Goals, Objectives:
Students successfully completing this course will be able to:
- Analyze the social drivers (determinants) of health (SDOH) affecting hospitalized patients and older adults in long-term care settings in rural and medically underserved areas to address critical gaps in care.
- Perform targeted physical assessments to identify changes from a patient's baseline clinical status and initiate appropriate, evidence-based nursing actions.
- Analyze standard diagnostic, laboratory, and physical assessment data to prioritize and implement safe care for hospitalized adults experiencing changes in health status.
- Develop comprehensive, evidence-based care plans for acute care patients and long-term care residents presenting with multi-morbidities, cognitive decline, and exacerbations of chronic illnesses.
- Collaborate with members of the interprofessional healthcare team to coordinate efficient patient care and advocate for necessary resources across acute and long-term care settings
- Demonstrate readiness to transition into a professional RN role within acute or long-term care settings in high-need geographic regions.

Attendance in all scheduled clinical activities is a critical component of student requirements for the HRSA: Workforce Readiness Grant. Attendance and active participation are essential for your learning and academic success. Students must attend all clinical dates. Excused and unexcused absences from clinical or simulation activities must be made up.
An Excused absence is: Verified illness of the student or a dependent family member Death or funeral in the family Court appearance Military obligations
Planned Absences: If you know you will be absent, you are required to notify the instructor via email as soon as possible and before the scheduled date of absence.
Extenuating Circumstances: In cases of emergencies or unforeseen circumstances that require your absence, please inform the instructor via email as soon as possible. Include details about the situation and, if appropriate, provide supporting documentation.
